Staff ride goes wrong. Very wrong.

I love when the Army uses clinical terminology to describe situations that were beyond-bonkers out-of-control. And then reported in the most military of newspapers.

“Not only is it questionable whether the purpose and intent of a (battalion staff ride) was met, but during the trip, multiple individuals exhibited lapses in judgment and leadership that are not expected of senior leaders in the Army,” the investigation stated. That's the kind of understatement you 1.) put into a briefing to higher and 2.) hope they won't ask too many question, but 3.) know they absolutely will.
